First Visit

New Patients

Your first visit is all about listening to your needs. Dr. Chitra will take your complete health history and perform thorough orthopedic, neurological, and postural exams. Based on her findings, she may refer you to an offsite facility for imaging if needed. After she has thoroughly evaluated your assessment and any imaging results, she'll develop a customized care plan. On your next visit, she'll present a full report of findings — including her analysis, your personalized treatment plan, and payment options.

Not necessarily — it depends on your situation. Dr. Chitra determines whether imaging is needed based on her exam and your health history during your first visit. X-rays are not done in-house; if needed, she will refer you to an outside facility. If you already have X-rays or other imaging from the past six months related to your area of pain, please bring them in so she can review them.

Your first visit includes a thorough consultation and evaluation. Dr. Chitra takes her time to understand your health history, perform a complete exam, and discuss your goals — so please plan accordingly. Completing your paperwork online beforehand helps save time.

Please bring a photo ID, your insurance card (if applicable), a list of current medications or supplements, and any imaging from the past six months related to your area of concern. We strongly encourage completing your forms online before your visit. All of our paperwork is 100% electronic — no printing or scanning needed. Yes, if Medicare is your primary insurance. However, we do not accept Medicare Advantage plans. If you're unsure about your plan, please call us at (904) 644-8100 and we'll help you figure it out.

We accept cash, credit cards, and personal checks. Payment is due at the time of service.
